Trauma Triage Algorithms:

The basic premise of getting the "Right Patient to the Right Place at the
Right Time"
was the foundation for the building blocks that make up
Colorado's Trauma System.

With this concept, Colorado's 11 RETACs are charged with using the State's
 approved "Trauma Triage Algorithm", and applying it to our regions. The
trauma patient's destination is a vital link in the EMTS system.

In 1995, The Foothills RETAC board carefully considered many factors when
designing our policies. Geographical differences, medical facility availability
and capabilities, specific EMS capabilities, and Medical Direction
preference within our RETAC were all considered. The FRETAC decided to
design county specific algorithms that would be EMS friendly for quick
reference and understanding, ALWAYS keeping in mind what is best for the
trauma patient.

For our New Trauma Triage Algorithm, we have tried to design an algorithm
that would work for ALL of our counties in one document. This has been a
really big challenge with our counties being very diverse. The draft has been
kept intentionally generic, but provides guidelines to our prehospital
providers.

Medical Direction:

The Medical Directors (Physician Advisors) for our participating EMS
agencies and facilities are a vital part to the decision making processes
within the Foothills RETAC.

We encourage their participation and involvement in all of the    
FRETAC activities from patient destinations to facility issues.

As we go along with the concept of CQI and developing our new Trauma
Triage Algorithms, it will be more important than ever for our Physicians
to be involved.

We invite all interested Medical Directors to join any and all of our
RETAC meetings. Your participation is much appreciated.

STATEWIDE CQI SYSTEM?:

The CDPHE EMTS Section has funded 10 of the 11 RETACs in Colorado for the
purpose of designing Regional CQI and Regional Medical Direction Systems. The
Program provides for all of our RETAC's 22 Prehospital Agency Medical Directors to
work together with the Mile-High Medical Directors to explore ways to look at QI and
prehospital issues throughout the two regions.

What does this mean for you and the RETAC?

  • The first thing to remember is that the QI Process will NOT identify individual
    agencies:

    All data will be de-identified and will not "Point Fingers" at individual agencies.

  • No Individual Physician will run this program!

    A GROUP of Docs from our RETAC that include Dr Peter Vellman, Dr Todd
    Dorfman, and Dr Jeff Beckman will invite ALL agency Medical Directors
    to participate. There is no "Authority" in this program to replace the
    individual agencies and their Medical Directors relationships.
    
    We hope it will provide the Docs and the agencies a
    forum for improving patient care through data!

  • Each Agency will receive reports. Reports can be designed to specifications

    Would you like to know how your agency (de-identified to others) compares
    with others for things such as:
  •        IV success rates
  •        Intubation success rates
  •        Medication Protocols and success rates
  •          Innovative ideas for patient care
  •        Etc, Etc, Etc...

The vision for a statewide/RETAC CQI System:

would include all of our prehospital providers, facilities, CDPHE, SEMTAC, and of
course our RETAC. It is currently seen as a circular system with patient data and
information flowing from a top down (CDPHE) to the Providers, and from the bottom
(Agencies and facilities) up with the RETACs acting as the funnel and regional
committee.

Get Involved!

The Foothills RETAC will be developing our CQI Program and Committee as we go
along in the process. The Clinical Care Committee is charged with issues relating to
patient care, so will probably act as the conduit as we develop our system. We invite all
stakeholders to stay involved or get involved as we move forward in the process.
